PACKAGE RECEIVED WITH DOSE RATES IN EXCESS OF ALLOWABLE LIMITS On March 17,1997, a GammaMed Source Changer containing 5 curies of iridium-192 was received at the CIS-US, Inc. facility located in Bedford, MA. The radiation level in one small area of the top surface of the package was 380 millirem per hour. This radiation level exceeded the 200 millirem per hour limit in 10 CFR 71.47(a). The radiation level at one meter from this surface was 3 millirem per hour. A CIS-US, Inc. licensee representative stated that the source was not secured in its shielded i
position when it was received. CIS-US subsequently moved the source to its shielded position and radiation levels were reduced to normal levels.

and from High Dose Rate Remote Afterloading machines. The GammaMed Source Changer was prepared for shipment at the Lahey Clinic Foundation, Burlington, MA (License No. 20-05766-02) on March 13,1996 by Frank Barker Associates, Inc., Towaco, NJ (License No. 29-28783-01). The package was then shipped by Federal Express on March 14,1996. Prior to shipment, the package was surveyed by Frank Barker Associates, Inc. and Lahey Clinic personnel. These surveys indicated 0.04 millirem per hour @
I meter and 2.0 millirem or less on contact with the package.
Region I is initiating a reactive inspection of this event.
The Commonwealth of Massachusetts and the State of New Jersey have been notified.
