Sealed Kyrpton 85 Source location Unknown On September 22, 1983, Valmet-Sentrol Incorporated personnel removed two sealed sources containing approximately 250 millicuries each of Krypton-85 from nuclear gauging equipment at the Fraser Paper Limited facility. The same day the sources were j packaged for shipment by Federal Express to Valmet-Sentrol Incorporated in Norcross,

! Georgia. On October 7, 1983, Fraser Paper Limited received a verift:ation notice from i Valmet-Sentrol Incorporated indicating that only one source was received NRC received initial notification of the incident at 1:45 p.m. on October 11, 1988. From discussions with Valmet-Sentrol Incorporated and Fraser Paper Limited, it appears that the package

was not damaged during shipment and that two source window covers for the sources were
received at Valmet-Sentrol Incorporated, but only one source was received.

1 i Fraser Paper Limited has been conducting a search for the krypton source and performing radiological surveys.

) The source is approximately 3/4 inch high and 3/4 inch in diameter and may present a j skin beta dose hazard if located directly against the body. Expected personnel j exposure rate at contact is. estimated to be between 1000 and 3000 rad per hour to the I

surface of the skin.

The States of Maine and Georgia have been notified of +.he missing source.

I Rogion ! is maintaining contact with the licensee and Valmet Sentrol to monitor recovery efforts. The State of Maine has been requested to perform confirmatory l surveys at the Fraser facility. The Region I PA0 is prepared to respond to media

] inquiries. The HQ Operations Officer has been informed.
