APPARENT LOSS OF SEALED SOURCE CONTAINING RADI0 ACTIVE MATERIAL On August 4, 1988, at 2:00 p.m. the licensee's acting Radiation Safety Officer (RS0) notified NRC Region I of the apparent loss of a calibration reference source. The apparent loss was discovered by licensee personnel during a routine physical inventory of sealed radioactive sources. The source is a small metal capsule containing 0.88 millicurie of cesium-137. The source is used for the calibration and testing of radiation detection and measurement equipment in the Nuclear Medicine Department.

Efforts by the licensee to locate the source have to date, been unsuccessful. The source was declared lost on August 4, 1988. On Friday, August 5, 1988, the licensee plans to release an advisory to all NIH employees on the Bethesda campus that describes the missing source and instructs employees to immediately contact the licensee s radiation safety office if the source is found.

One millicurie of cesium-137 is generally considered not capable of causing a lethal radiation dose. Prolonged exposure at close contact to this quantity of radioactive material could result in localized burns.

The State of Maryland has been notified.

NRC Region I will maintain close contact with the licensee's radfation safety staff concerning this matter.
