REPORTED WHOLE BODY OVEREXPOSURE On October 11, 1985, the licensee's Radiation Safety Officer contacted NRC Region I by telephone to report that one of the licensee's radiographers had received a whole body dose of 15.5 rem, as determined by the licensee's film badge supplier, R. S. Landauer, Incorporated. The licensee representative stated that the incident occurred at 9:50 a.m. on October 8, 1985 at Dresser Industries in Olean, New York. The radiographer apparently failed to perform a radiation survey af ter a radiographic exposure and set up the subsequent exposure without having retracted the 100-curie iridium-192 source to its shielded position in the exposure device. The radiographer was removed from radiation

, work for the remainder of the quarter. The Radiation Safety Officer said that he would promptly inform the State of New York.

Since this exposure occurred in New York, an Agreement State, any further investigation and/or enforcement actions will be taken by the State. Region I has informed the State of this incident.
