Susquehanna notified the NRC of a permit violation via EN 50690 on December 17, 2014. The required report submitted to the Pennsylvania Department of Environmental Protection associated with the violation is attached.

This letter contains no new regulatory commitments.

Attached please find the Susquehanna Steam Electric Station's (SSES) completed Si;:initary Service Overflow (SSO) form. This completed SSO form summarizes what SSES presently knows about the sewage leaks discovered on SSES property on 12/17/14.

NOTE: Submittal of this completed SSO form to PaDEP satisfies SSES's follow-up reporting obligation specified under NPDES Permit No. PA 0047325.

The other leak is located on the west side of the same agricultural field.

Permittee hired a contractor (PA Environmental Solutions, Inc.) to proactively perform a line By who?

trace of the forced main leading from the SSES Environmental Lab to the SSES Sewage Treatment Plant. Leaks were discovered while contractor was performing the line trace.
