CLOSURE OF INVESTIGATION (OI 4-2022-025)
Dear Ken Peters:
This refers to an investigation conducted by the U.S. Nuclear Regulatory Commissions (NRCs)
Office of Investigations regarding activities at Comanche Peak. The Office of Investigations initiated the investigation to determine whether licensed operators and non-licensed operators failed to report or declare fatigue and were inattentive while on shift.
Based on the evidence developed during the investigation, the concern that licensed and non-licensed operators failed to report or declare fatigue, and were inattentive while on shift was not substantiated. The NRC plans no further action related to the alleged wrongdoing and considers this investigation closed.
