SUMMARY
OF FITNESS FOR DUTY ACTIVITIES FOR JULY THROUGH DECEMBER 2004 POSITIVE RESULTS:
Four drug tests produced positive results during this reporting period.
PRE-ACCESS:
There were four pre-access drug tests with positive results during this testing period.
Two tested positive for amphetamine, one for THC, and one for cocaine. All individuals were contract employees; they were denied unescorted access in accordance with company procedure.
RANDOM:
There were no positive results during this reporting period.
FOLLOW-UP:
There were no positive results during this reporting period.
FOR-CAUSE/POST ACCIDENT:
Five for-cause/post accident tests were performed during this reporting period with no positive results.
- One individual was tested after receiving a pinch on the hand.
- Three individuals were tested after vehicle accidents.
- One individual was tested after falling from scaffolding.
FOR-CAUSE/OBSERVED BEHAVIOR:
Twenty for-cause/observed behavior tests were performed during this reporting period with no positive results.
- Four individuals were tested after alcohol was discovered in their vehicle at the owner controlled area access control point. All tests were negative.
- Three employees were tested after a report of an odor of alcohol. All tests were negative.
- Three employees were tested after inappropriately positioning a piece of plant equipment. All tests were negative.

OTHER:
One individual requested a test after an allegation was lodged against him. The test was negative.
RANDOM TESTING RATE:
The random testing rate for this period was 27.4%.
The annual random testing rate for 2004 was 58.7%.
