EVENT:

2004-01 CAUSE:

Scheduled Maintenance Outage with Generator off-line while Reactor at approximately 30%

power to replace a power supply In the Generrex panel.

CORRECTIVE ACTION:

Replaced power supply In Generrex panel CRITICAL PATH WAS:

Replacing power supply in Generrex panel and synchronizing generator to grid.

EVENT:

2004-02 CAUSE:

Forced Outage due to a Manual Reactor Trip on loss of Feedwater Pump Speed Control.

CORRECTIVE ACTION:

Repaired Feedwater Pump Steam Flow Control Valve which had been sticking and repaired the Main Feedwater Regulating Valve which would not dose. Performed prerequisites for mode changes.

CRITICAL PATH WAS, Repairing the Feedwater Flow Control Valve NARRATIVE REPORT March 01 00:00 Unit I at 100% power with no significant operating problems.

March 26 20:50 Unit I began derating for a planned maintenance outage with reactor to remain at approximately 30% power.

March 27 04:56 Unit I Turbine tripped to support maintenance activities.

March 27 21:10 Unit 1 Generator synchronized to the grid following maintenance on the generrex system.

March 27 22:05 Unit 1 Reactor was tripped manually due to the loss of feedwater pump speed control.

March 29 05:48 Unit 1 Entered mode 2.

March 29 06:54 Unit 1 Reactor is critical.

March 30 18:23 Unit 1 Entered mode 1.

March 30 23:13 Unit 1 Generator synchronized to the grid.

March 31 23:59 Unit 1 at approximately 76% power ramping up after the forced outage.
