The results of the annual fatigue management program effectiveness review determined Wolf Creek is effective in controlling the work hours of individuals subject to work hour controls.

The Corrective Action Program is adequately used to identify and correct concerns with compliance to the fatigue rule requirements. All fatigue assessments did have a condition report initiated to be reviewed through the corrective action program.

The implementation of this training included a change in the process when donors enter the collection room, on hands training with examples for collectors to identify synthetic urine, Industry OE sent to all collectors for review, and reviewing yearly statistics from around industry.

Since the implementation, Wolf Creek has improved catching subversions. In 2024, 2 subversions were identified. This year the number of subversions identified was 7. Wolf Creek presented this training at the NEI Access Authorization Fitness For Duty Annual Conference. Following this presentation, Wolf Creek was asked to present this training to an industry peer on one of their sites for several of their plants.

Topic 2: Program and System Management Wolf Creek created multiple reports to assist with managing the Follow-up Program. One report is generated automatically every day. This report puts all information needed to manually track follow-up programs in one place. It has been invaluable to ensure we do not mismanage an individuals program and potentially over test or under test them, leading to a violation of the individuals program and the 10 CFR 26 regulation. We created an additional report that we can run at any time to monitor the Follow-up programs as well. This report also puts everything in one place to be able to successfully monitor and execute the Follow-up programs.

Topic 3: Program and System Management Wolf Creek successfully trained and implemented an Interim FFD Program Owner while the current program owner completed a loanee program with NEI for 7 months. The interim program owner fulfilled any and all program elements for 7 months successfully, they had no policy or procedure violations, no regulation violations, and no other minor issues.

The collector heard sounds that sounded like a bottle being opened and moved around on the shelf but no sounds of urination. The individual was in the restroom for a few minutes. Never heard any sounds of urination, not into a cup or into the toilet. When they opened the door, the collector placed the temperature strip on the specimen cup, and reported it registering between the 94-96 range. Access Screening received information from Employee Concerns that this individual was asking for clean urine for their pre access test that was going to be the next day. Individual was conducting training, brought to FFD for a For Cause Collection. preformed the first collection. Specimen was at the exact 60 ml amount, specimen had no smell, it sounded like a plastic cap was opening while they were in the restroom, and her specimen was barely registering a temperature.

Access Screening received information from Employee Concerns that this individual was asking for clean urine for their pre access test that was going to be the next day. Individual was conducting training, brought to FFD for a For Cause Collection. preformed the first collection. Specimen was exactly 60ML, no bubbles, Temp 90-92, and no odor. A direct observation collection was requested.

When the individual checked in they were notified they would have to conduct a FFD test. They stated they didn't feel well and left. They were called multiple times to come back to preform the collection and then go home. They never showed up. The individual was contacted and stated they would come back to site, but never showed.
