Interim Report of a Deviation or Failure to Comply Associated with General Electric HMA Type Auxiliary Relay, Model 12HMA124A2, Part Number DA137C6164P001 In accordance with the requirements of 10 CFR 21.21 (a)(2), James A. FitzPatrick Nuclear Power Plant (JAF) is submitting this interim report for General Electric (GE) HMA Type auxiliary relays, model 12HMA 124A2, part number DA 137C6164P001.
A sample of HMA Type auxiliary relays was sent to Powerlabs for Parts Quality Initiative (PQI) testing. The results were reviewed by JAF on November 25, 2025, and a deviation in two relay components was discovered. Testing identified in one component a failure of contact 8-4 to close and in the other component the contact 8-4 functioned inconsistently.
The effect of this deviation is a failure of the relay to change states when de-energized.
The cause of the deviation is under investigation, and the results will be incorporated into the evaluation required by 10 CFR 21.21(a)(1). This is expected to be completed by April 10, 2026.
