From December 15-18, 2025, the U.S. Nuclear Regulatory Commission (NRC) staff conducted a safety inspection at your University of Utah TRIGA Nuclear Reactor facility. The inspection results were discussed with you and the Reactor Supervisor on December 18, 2025. The enclosed report documents the inspection results.
The inspection examined activities conducted under your license as they relate to safety and compliance with the Commissions rules and regulations and with the conditions of your license. The inspector reviewed selected procedures and records, observed various activities, and interviewed personnel. Based on the results of this inspection, no findings of significance were identified. No response to this letter is required.

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an inspection at the University of Utah TRIGA Mark-I nuclear reactor facility, in accordance with the research and test reactor inspection program. This is the NRCs program for overseeing the safe operation of licensed research and test reactors. Refer to https://www.nrc.gov/reactors/non-power.html for more information.

3 REPORT DETAILS Facility Status The University of Utah 100 kilowatt (kW) TRIGA Mark-I nuclear reactor (UUTR) was typically operated in support of educational demonstrations, laboratory experiments, reactor system testing, sample irradiations, and operator training. It was usually operated one or two days a week at various power levels up to 90 kW. During the inspection, the reactor was operated to complete the annual operations examination for the Facility Director, a Senior Reactor Operator.
Inspection Scopes Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at https://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Inspections were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter 2545, Research and Test Reactor Inspection Program. The inspector reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.
69001 - Class II Non-Power Reactors This inspection included a review of selected aspects of the research and test reactor, since the last inspection, to verify activities were conducted safely and in accordance with regulatory requirements and licensee commitments.
Organization and Staffing (IP Section 02.01)
This inspection included a review of the licensees organizational changes and shift staffing records to verify compliance with the technical specification (TS) requirements and licensee commitments. Specifically, the inspection included a review of the following:

Specifically, the inspection included a review of the following:

This inspection included a review of selected documents pertaining to operator requalification training to verify requirements of the licensees approved requalification plan and Title 10 of the Code of Federal Regulations (10 CFR) Part 55, Operators Licenses were met. Specifically, the inspection included a review of the following:

Limiting conditions for operation were also reviewed to verify that operating conditions were maintained in accordance with the licensees procedural requirements. Specifically, the inspection included a review of the following:

The inspection also verified that modified equipment would continue to meet regulatory and procedural requirements and licensee commitments. Specifically, the inspection included a review of the following:

2019-202-01 Follow-up on the licensees actions to complete the revision of the E-Plan and have it reviewed and approved by the RSC.
IP 69001, Section 02.09 Section 50.34, Contents of applications; technical information, of 10 CFR requires in paragraph (b)(6)(v) that each application for an operating license shall include a final safety analysis report. The final safety analysis shall include plans for coping with emergencies The University of Utah, Utah Nuclear Engineering Program (UNEP) E-Plan, dated June 2011, requires in section 10.4, Emergency Plan Review and Update, that the RS
[Reactor Supervisor] shall periodically revise and update the Emergency Plan and the RSC [Reactor Safety Committee] shall review and approve the overall UNEP Emergency Plan to ensure that the Plan is adequate and current, and shall specifically review and approve any revisions to the Emergency Plan that affect implementation procedures.
In August 2019, an inspector noted that the RSC did not complete a biennial review of the E-Plan. This was because a revision of the E-Plan to incorporate suggested changes was not finished. The licensee indicated that the revision was in progress and would be ready for review by the December meeting of the RSC. The licensee was informed that the revision of the E-Plan, and review and approval by the RSC, would be designated as an IFI.
During an inspection in 2024, the inspectors reviewed the issue and found that the E-Plan was revised various times since 2019 and a review by the RSC was still pending. Because there were numerous changes in the Facility Director and Reactor Supervisor positions since the issue was first noted, the licensee has not followed this issue properly and no review by the RSC has occurred as required. This issue is noted as a violation of an administrative requirement in the E-Plan and does not adversely affect the public health and safety. This failure to comply with the University of Utah E-Plan, section 10.4 constitutes a minor violation that is not subject to enforcement action in accordance with the NRC Enforcement Policy.
Planned Closure Actions: The Reactor Supervisor indicated that the E-Plan would be revised and presented to the RSC by the next meeting of the committee.
Licensee Actions: During this inspection, the inspector noted that the E-Plan was revised further and submitted to the RSC on March 21, 2025. The RSC reviewed and approved the latest revision (Revision 9) of the E-Plan. The E-Plan was submitted to the NRC for information. This issue is considered closed.

2021201-02 Follow-up on the licensees actions to revise procedure UNEP-002R4, Biennial Fuel/Tank/Control Rod/Reflector Element Inspection, to include maintenance actions needed during control rod inspection.
IP 69001, Section 02.03 The inspector reviewed the circumstances surrounding a Special Report issued by the licensee, dated April 2, 2020, in accordance with UUTR TS section 6.7.2 item 1.5 documenting a component malfunction. On March 30, 2020, facility personnel completed the routine monthly inspection of various reactor components and systems and initiated a standard reactor startup and termination checklist to test all systems. One portion of this test was to withdraw the safety control rod to 100 percent and then disrupt the power to the magnet causing the rod to drop. The licensee staff noted that the safety rod did not drop as expected.
After further review, the safety rod was disassembled, and rust and calcium deposits were noted and subsequently removed. Following these corrective actions, the safety rod was inspected, reassembled, and reinstalled in the core. Testing and calibration of the safety rod indicated that the malfunction was resolved, and the rod functioned properly.
During an inspection in 2021, the inspector reviewed this issue. In their letter to the NRC concerning the component malfunction, the licensee indicated that the procedure involving the inspection of the control rods, UNEP-002R4, Biennial Fuel/Tank/Control Rod/Reflector Element Inspection, would be revised to include checking for debris, binding, rubbing, or misalignment of the control rod drives that might prevent the control rods from dropping. The licensee was informed that the revision of the procedure would be considered an IFI. During an inspection in 2023, the inspector determined that this revision was still pending.
Planned Closure Actions: The Reactor Supervisor indicated that the procedure would be revised and presented to the RSC by the next meeting of the committee.
Licensee Actions: The licensee presented the revised procedure to the RSC and on July 18, 2023, the RSC approved the procedure. The revised procedure was required to be read by all reactor operators and operator trainees. This issue is closed.

2023201-02 Follow-up on the reactor tank repair and the complete stoppage of tank water loss due to leakage.
IP 69001, Section 02.11 The inspector reviewed the circumstances surrounding the notification made by the licensee on March 31, 2023, to the NRC regarding a reactor pool leak at the UUTR. The UUTR staff personnel noted the reactor tank developed a leak of approximately 24 gallons per day. The licensee noted that there was no indication of leakage to the environment and the leakage did not reach or exceed any reporting criteria. However, a few days later the licensee noted that the leak rate increased to about 50 gallons a day. Water samples taken when the leak rate increased continued to show there were no activity levels that reached any reporting criteria or above any regulatory limit.
The licensee determined the most probable leak site would be a degraded weld presumably toward the bottom of the circumference of the tank (the point of highest pressure). To resolve the leak problem, the licensee hired a contractor to apply a two-part epoxy sealant material to the weld areas around the bottom three sections of the tank. During an inspection in July 2023, the inspector observed the repair work on the tank. At the completion of the inspection, the inspector noted that leak appeared to have stopped but that the success of the repair project would need to be evaluated further during a future inspection. The licensee was informed that the issue of the reactor tank repair and the complete stoppage of tank water loss due to leakage would be considered an IFI. During an inspection in 2024, the inspectors noted that the original problem with a leak at a weld seam was apparently resolved. However, after approximately one month, a new leak developed, and the licensee had to take further action to repair the reactor tank a second time.
Planned Closure Actions: The licensee contacted a new contractor, and it was decided to remove the reactor fuel from the tank, drain the tank, sand blast the entire inside of the tank, patch any holes noted with aluminum patches, and apply a different type of epoxy coating to the entire interior surface of the tank.
Licensee Actions: A second contractor was hired and the items mentioned above were completed in July 2025. Beginning in August, the reactor and fuel storage racks were installed, biennial fuel inspections were completed, other instruments were installed, the control, shim, and reg rods were installed, and approximately 800 gallons of water were added to the tank. The various required calibrations and other surveillances were completed as well. The two Senior Reactor Operators completed the required operator training and reactor manipulations at another facility, completed annual written examinations, completed annual oral examinations, and completed UUTR reactor operations as required by the NRC.
The reactor is considered operational and the reactor operators are qualified to operate the reactor and train others. The reactor tank leak and repair issue is considered closed.
Exit Meeting and Debriefs The inspector verified no proprietary information was retained or documented in this report.
On December 18, 2025, the inspector presented the NRC inspection results to Dr. Edward Goodell, Director and Andrew Allison, the Reactor Supervisor.
