1 APPENDIX B - Part 1 (Sea Turtle)

Incident Report Sea Turtle Take - PSEG Salem Generating Station Photographs should be taken and the following information should be collected from all turtles and sturgeon (alive and dead) found in association with Salem. Please submit all turtle necropsy results (including sex and stomach contents) to NMFS upon receipt.

Fate of animal (circle one): dead alive Condition of animal (include comments on injuries, whether the turtle is healthy or emaciated, general behavior while at Salem.

During routine inspection of Circulating Water Intake bays, Salem Operations personnel identified a deceased sea turtle impinged on the trash rack of 13B intake. The operator notified the Environmental Consulting Services Inc.

(ECSI) fisheries technician on-site at the time, and Station Security was notified to assist with access to the intake for retrieval. Specimen was removed and identified as a Kemps ridley sea turtle. Salem Unit Control and PSEG Nuclear Environmental Affairs personnel were notified. After taking possession, ECSI personnel began processing of the specimen.

2 APPENDIX B, continued (Incident Report of Sea Turtle Take)

Draw wounds, abnormalities, tag locations on diagram and briefly describe below.

Description of animal:

Specimen was in poor condition at the time of retrieval. While no significant external injuries indicating a likely cause of death were found, the specimen revealed indicators of early decomposition, such as foul odor, slight bloating, and degradation of skin and scutes on the carapace and head (area identified by red circle), as well as abrasions to skin, ventrally, on the upper portions of the hind flippers. External tags were not found, and an internal PIT tag was also not indicated.

3 Salem SGS Incidental Take Report - Sea Turtle Dorsal view of the deceased, Kemps ridley sea turtle, collected from Salem Generating Station CWI on 10-13-2025 by Salem Operations personnel during routine trash rack inspections (10/13/2025).

A ventral view of the specimen, showing the condition of the plastron. Except for two skin abrasions to the upper portion of the hind flippers, no significant signs of injury were observed. In addition to these observations, indication of early decomposition was also noted (10/13/2025).

4 Salem SGS Incidental Take Report - Sea Turtle The existence of distinct pores (circled in red), near the margins of the inframarginal scutes, were observed on both sides of the plastron, confirming the identity of the specimen as a Kemps ridley sea turtle [10/13/2025].
