Dear Sir or Madam:
In accordance with 10 CFR 70.74(b), Global Nuclear Fuel-Americas, LLC (GNF-A) hereby submits a written follow-up report for Event Notification 57884 that was provided to NRC on August 22nd, 2025 (Reference 2).
Event 57884 As discussed in the initial event report, GNF-A reported an unplanned fire suppression system impairment to the New Hanover County (NHC) Deputy Fire Marshal. Consistent with 10 CFR 70, Appendix A(c), a concurrent report was provided to NRC and is included as Attachment 1 to this letter.
Additional information is provided as follows:
Event Details and Safety Significance At approximately 1035 (EDT) on August 22nd, 2025, the New Hanover County Deputy Fire Marshal was notified per North Carolina Fire Code requirements that a fire suppression system riser, which provides sprinkler protection to a portion of the fuel manufacturing operation (FMO),
was impaired due to missing ceiling tiles. A fire watch for the affected area was initiated.
Immediate Corrective Actions Taken A fire watch was initiated and repairs begun.
USNRC M250338 October 20, 2025 Page2 Probable Cause of Event An investigation determined the probable cause to be a water leak from an overhead HVAC unit which damaged some ceiling tiles and caused them to fall to the floor.
Short Term Corrective Actions A fire watch was immediately initiated for the affected area and identified missing ceiling tiles were replaced. The leak in the overhead was addressed by shutting off the chilled water valve to the HVAC unit and removing the unit from active service. Completed 10/20/2025.
Longer Term Corrective Action None.
