Submittal of Written Exam Results and Post Exam Items for Initial License Class 2025
Dear Mr. Hernandez,
Enclosed you will find the written examination supplemental materials that were administered on 3/19/2025 at Callaway Energy Center.
The exam item analysis performed showed that Questions 30, 53, 81, 83, 91, 92, and 97 were high miss questions (~50% failure rate). The results of the exam item analysis found no question flaws with any of these questions. Training Requests were generated as stated further below.
An exam review was held on 3/20/2025 with the 2025 applicants which resulted in challenges to Question 77 because it contains an unclear stem that confused the applicant.
The facility's recommendation is to remove Question 77 from the examination on the basis that per NU REG 1021 Rev 12, ES-4.4 C.3.c first bullet, "a question with an unclear stem that confused the applicants or did not provide all the necessary information" is grounds for NRC consideration for exam changes. Per ES-4.2 section O.15.a, distractors should follow grammatically from the stem. In this case the keyed answered and 2 distractors did not follow this rule.
The stem of the question asks, "What actions are required per Tech specs?". Three of the four available answers, including the correct keyed answer, only provide ONE action, not two or more as indicated by the stem (by the word actions). The applicant who answered incorrectly (based on the key) selected the one available answer that provided TWO actions which grammatically follows what the stem asked.

This difference in plural vs singular tense resulted in the applicant misunderstanding the intent of the question and the validity of the answer choices, leading the applicant to select the only choice with two actions.
Proper stem wording would have been "What action(s)... " The question has been revised in Callaway's Exam Bank.
The issues associated with Questions 77 were not recognized during exam development or validation.
CR202501281 was written to address the Area Focus item for Low Power Operations as discussed in the 2025 License Exam Debrief. Course Enhancements 1030, 1031, and 1032 were written to incorporate the approach to criticality and POAH into Off Normal and Emergency Operations Simulator Scenario training, as well as more hands on training with Digital Feedwater Controls under those conditions.
Course enhancements 1036 and 1037 were generated to address issues noted during Exam Development concerning updated ESFAS actuation setpoints and Radwaste procedure instruction for the relevant IL T Lesson Plans. Additionally, 5 separate Training Requests (TR2025118/119/120/121/122) were generated to evaluate ILT program enhancements based on the high missed questions.
