NRC Letter to Entergy, "Grand Gulf Nuclear Station - Integrated Inspection Report 05000416/2024004 and Notice of Violation," dated February 13, 2025 In accordance with 10 CFR 2.201, Entergy Operations, Inc. hereby submits this Reply to a Notice of Violation; for Grand Gulf Nuclear Station. As requested, the Enclosure contains the following: (1) the reason for the violation; (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken; and (4) the date when full compliance will be achieved.

This letter contains no new commitments and no revisions to existing commitments.

G N RO2025-00006 Enclosure Page 1 of 2 Reply to a Notice of Violation NRC Inspection Report 05000416/2024004 described a Green Notice of Violation by Entergy Operations, Inc. (Entergy) at the Grand Gulf Nuclear Station (GGNS) as restated below:

Title 1 O CFR 20.1501 (c) requires that the licensee shall ensure that instruments and equipment used for quantitative radiation measurements (e.g., dose rate and effluent monitoring) are calibrated periodically for the radiation measured.

Contrary to the above, from at least April 2018 to December 2, 2024, the licensee failed to ensure that instruments and equipment used for quantitative radiation measurements (e.g., dose rate and effluent monitoring) were calibrated periodically for the radiation measured. Specifically, the licensee failed to periodically calibrate and maintain the drywell personnel airlock ARM in accordance with 10 CFR 20.1501 (c).

Entergy does not contest the enforcement action and concurs that the drywell personnel airlock area radiation monitor has not been calibrated periodically in accordance with 1 O CFR 20.1501 (c).

Pursuant to the provisions of 10 CFR 2.201, this Enclosure provides Entergy's Reply to a Notice of Violation.

Response by Entergy

1) Reason for the Violation After the 2022 radiation monitoring instrumentation inspection a violation was identified where GGNS had discontinued periodic calibration of 34 area radiation monitors. In response to the violation, GGNS reinstated preventative maintenance frequencies to all area radiation monitor calibrations. While the frequencies were reinstated, a plan was not put in place to immediately calibrate the area radiation monitors. This resulted in the calibrations being scheduled to occur based on the newly-assigned frequency which caused the drywell personnel airlock area radiation monitor calibration to neither be scheduled nor occur during the 2024 refuel outage.
2) Corrective Steps That Have Been Taken and the Results Achieved The following corrective steps have been taken:

A Work Order has been created to ensure calibration during the Spring refuel outage scheduled for 2026. Due to the location, the drywell personnel airlock area radiation monitor must be calibrated during an outage of sufficient duration.

A preventive maintenance activity is in place to assure regular periodic calibration of the drywell personnel airlock area radiation monitor.

3) Corrective Steps That Will Be Taken The remaining corrective step to be taken is calibration of the drywell personnel airlock area radiation monitor. The drywell personnel airlock area radiation monitor will be calibrated during the next refuel outage.
4) The Date When Full Compliance Will Be Achieved Full compliance will be achieved following calibration of the drywell personnel airlock area radiation monitor. This will occur prior to the drywell closeout at the end of the next refuel outage currently scheduled to occur in the Spring of 2026.
