LICENSEE:
The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the
Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective
examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings
are as follows:

17. Scope and Observations:
This was an announced field inspection of a State of Louisiana licensee (general licensee under 10 CFR 150.20)
authorized to fabricate sealed sources used in HDR brachytherapy units, perform service operations including
installation, maintenance, repair, source exchanges, and removal from service for devices specified on its
Agreement State license. The inspection was limited to observations of the licensees source exchange service
activities performed on the customers Leksell Perfexion GSR unit at the temporary job site.
This inspection consisted of interviews with select licensee personnel; a review of select records (including radiation
safety and operating procedures); and observations of licensee staff performing source exchange operations within
the customer's facility. The inspector observed the use of personnel monitoring (extremity, whole body and digital
electronic dosimeter), sealed source leak tests, and surveys. The service representatives demonstrated their
knowledge in the licensee's operating and emergency procedures, site specific tasks, stopping points, and
responsibilities during the service activities. The inspector performed periodic independent surveys of the portable
hot cell, the shipping cask and the GSR unit during the course of the source exchange operations. A side-by-side
comparison of the licensees survey instrument and the inspectors instrument (RadEyeG, SN 30653, calibration
date, 5/14/2024) was made on the surface and at three feet from the shipping cask containing the new sources; both
instruments were within 20% agreement.
No violations of NRC requirements were identified during this inspection.
