SPECIAL REPORT 2025-002 INOPERABLE RADIATION MONITORING INSTRUMENTATION CHANNEL Dominion Energy South Carolina, Inc. (DESC) hereby submits Special Report 2025-002, pursuant to the requirements of Technical Specification (TS) 6.9.2, as required by Action 30 of TS 3.3.3.1.b, Table 3.3-6, RADIATION MONITORING INSTRUMENTATION, Item 2.d.i.

The Main Plant Vent Radiation Monitor, RM-A13, was declared inoperable on January 7, 2025, due to an instrument trouble alarm. RM-A13 provides extended range backup radiation monitoring to the RM-A3 functions for the Main Plant Vent described in Final Safety Analysis Report (FSAR) Section 9.4.2, Auxiliary and Radwaste Area Ventilation System. RM-A13 is described in FSAR 11.4, Process and Effluent Radiological Monitoring Systems, and its associated tables and figures, as well as Table 12.2-3 and Figure 12.2-2.

Upon declaring RM-A13 inoperable, VCSNS implemented a preplanned alternate method of monitoring in accordance with the requirements of Action 30 of TS Table 3.3-6. Troubleshooting determined that a new detector and amplifier were required.

The new instrumentation has been obtained, assembled, and calibrated. VCSNS is currently in the installation/testing phase of the replacement and expects to restore RM-A13 to OPERABLE status by February 20, 2025.
