License Amendment Request Ref: NBSR Facility License TR-5, Docket 50-184 Sir/Madam:

On November 22, 2024, NCNR personnel identified that the existing text in the NBSR Technical Specifications (Amendment 15, March 2, 2023) Table 3.2.2 and Table 4.2.2 might (inadvertently) permit operations of the NBSR when either one of the inner or outer plenum flow channels is bypassed.

Further investigation revealed that the technical basis for low-flow Technical Specification requirements and their associated safety analyses require both plenum flow channels to be operational.

As a result, the NCNR has identified a necessary change in the Technical Specifications to revise operability requirements for primary coolant flow channels. On 11/26/2024, NCNR took immediate action in the form of Shift Supervisor Instructions (SSI 74), and specific training for reactor operators was completed on 11/26/2024. NCNR is hereby submitting the attached 30-day written report to NRC in accordance with NBSR Technical Specification 6.7.2(2)(b) in the form of Engineering Change Notice (ECN) #1399. In addition to the interim corrective actions, we are also submitting a License Amendment Request, given in ECN 1399, to modify NBSR Technical Specifications 3.2.2 and 4.2.2 to require that both inner and outer plenum flow channels be operable during reactor operations.
