Response of James Cory Bradshaw to Apparent Violations, NRC Investigation Report 4-2023-016; IA-24-009
Dear Mr. Miller:
Please allow this correspondence to serve as the response of James Cory Bradshaw to the above referenced Apparent Violations.
A. Apparent Violation of 10 C.F.R. Section S0.5(a)(2).
The Factual Summary states in pertinent part that "[o]n April 17, 2023, (Mr. Bradshaw] reported to a site nurse that (he] had stopped taking [his] prescribed medications about 2 weeks prior[,]" and that "[o]n April 20, 2023, [Mr. Bradshaw] revised [his] statement and told the site nurse that [he] stopped taking the medications in January 2023." The Factual Summary further states that "[o]n May 2, 2023, [Mr.
Bradshaw] reported to [his] supervisor that [he] had actually stopped taking the medications in January 2022 without consultation or direction of a physician."
Mr. Bradshaw does not dispute the above facts. While not an excuse for going off the medication regimen and not reporting the same, Mr. Bradshaw believed the medications were making him feel physically ill and giving him indigestion. It did not initially occur to Mr. Bradshaw that his change to his medication regimen was something that he needed to report, and by the time it did occur to him that the change did need to be reported, he was afraid of the repercussions of having failed to report the change sooner.
