Notice of Violation, EA-24-096:

NOTICE OF VIOLATION RESPONSE DocketNo.07100062 EA-24-096 Contrary to the above, as of September 2021, Industrial Nuclear Company, Inc. used measuring and test equipment (M&TE) that was not properly calibrated for activities impacting quality. Specifically, the receipt inspection reports for the outer body pipes of the package, categorized as important-to-safety category B, did not document that the pipe diameters were measured using properly controlled M&TE. The pipe diameters were identified as a critical characteristic. One receipt inspection report, which involved five outer body pipes, indicated that a tape measure was used which cannot be accurately calibrated and was incapable of measuring to the required tolerance for the pipe dimensions. In addition, another receipt inspection report did not list the M&TE used to measure the critical dimension.

Response Format:

"Reply to a Notice of Violation, EA-24-096" and should include for the violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation or severity level; (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken; and (4) the date when full compliance will be achieved.

Response to Notice of Violation, EA-24-096:

INC concurs with the violation as described in Notice of Violation EA-24-096 and has taken the following corrective actions. INC issued Corrective Action Report (CAR) 24-02 and Nonconformance Report (NCR) 061 on 7/15/2024. CAR 24-02 identified a failure to properly implement QP 12.1 for the selection and use of M&TE with suitable range and accuracy for the items being measured. All employees involved in this process received training to the requirements of QP 12.1. The QA Manager purchased a measuring device with range and accuracy suitable for this application from a company that was added to IN C's Approved Suppliers List for calibration of this device.

Actions taken in response to NCR 061 included locating two representative samples of the ten OP-RMSCs that were produced. The outer body pipes were all cut from the same heat of material. INC measured the two pipes with the calibrated device and the pipes were found to be acceptable. The device's accuracy is 0.01 "which exceeds the accuracy requirements of 0.03" on the related design drawing OP-RMSC-OB-1.

Based on the above actions both NCR 061 and CAR 24-02 were closed.
