INSPECTION PROCEDURE 88072 - PLANT MODIFICATIONS (Triennial)

FOCUS AREAS (in order by SAR number):

  • SAR 15.14, Scrubber Ventilation and Vacuum System in Uranium Recovery
  • SAR 15.32, Pharmacy Operations SAR 15.37, Higher Tier Assemblies (with focus on the Pickling area)

NOTE: Since the requested documents below could be classified or otherwise challenging to upload into an electronic reading room, please place the requested information in the NRC senior resident inspectors office. Region II will send an inspector to perform a bagman trip (the week before the inspection) to review, evaluate, and prioritize the plant modifications (mods) to inspect.

We will start with the one we deem with the most safety significant mods and conduct an intensive review. Time permitting, we will review the second most important safety significant mods and so on. It is possible that we will only cover one of these mods.

1. Information Requested for In-Office Preparation (Please leave in residents office)

A. Engineering/Configuration Management organization chart B. List of system engineers and assigned systems for the focus areas above C. A list of plant modifications program-related procedures including post-mod testing.

Include procedure number, title, and revision #/version #.

D. The focus of the inspection will be on mods completed in the focus areas above between start-up until 2024. Please provide a list of mods in the area that covers this timeframe.

NOTE: Please provide this list ASAP so that we can provide you with the specific mods that will be requested for on-site review. IF this list too extensive, you may limit the list to significant modifications including ones that impacted the ISA or safety bases.

E. List of current outstanding design issues (including open/deferred or canceled engineering change items), temporary modifications, and operator workarounds for the focus areas above F. List of active corrective maintenance work requests/orders for the focus areas above

2. Information Requested for On-site Review A. Provide the following documents for the focus areas above, as applicable:
a. Piping and Instrument diagrams
b. Electrical single line diagrams
c. Process Hazards Analysis (PHA)
d. System descriptions
e. Operational procedures
f. ISA Summary/SAR (various revisions upon request)
g. All modifications/changes related to the focus areas above B. List of condition reports for last 12 months pertaining to PM/engineering program activities for the focus areas above C. List of open condition reports associated with the focus areas above
3. Onsite Coordination A. Please schedule a general plant tour of the focus areas above after the entrance meeting or early Tuesday.

B. Meeting with the engineer(s) and appropriate personnel for the focus areas above in order to discuss major changes (physical and SAR-related)
