Request for Additional Information Regarding Reciprocity Request Discussion:
I contacted Mr. Taylor because of discrepancies between the cover letter and the NRC Form 241 that was submitted. Mr. Taylor stated that he was requesting reciprocity for the remainder of the year. I informed Mr.
Taylor that an initial request for reciprocity had to be made three days prior to the first day of work so I could not authorize any day prior to August 17th. I requested that I be provided the names and contact information for the three requested work sites. Mr. Taylor stated he did not have that information at this time and that the request was made in case licensed activities should become necessary. I informed Mr. Taylor that I could not enter the information about the work sites at this time because he did not have the contact information. I would record that determination in the cover letter, on the NRC Form 241 and in a telephone record. When he had the required information and sent in an NRC Form 241 with the changes, I would enter the information into our database and approve the licensed activities.
