TRANSFER OF SEALED SOURCE AND DEVICE REGISTRATION FROM NEW YORK TO THE NRC, CERTIFICATE NO S. NR- 0502-D-107-G, NR-0502-D-108-G, AND NR- 0502-D-109 -G
Dear Mr. Colon-Gonzalez:
This is a follow-up to the letter which Ms. Booma Venkataraman, U.S. Nuclear Regulatory Commission (NRC), sent to you, dated September 29, 2023, informing you that the NRC would issue new sealed source and device (SS&D) registration certificates to replace the ones which had been issued by the New York State Department of Health (NYSDOH).because the authority to issue SS&D registrations has been transferred from the NYSDOH to the NRC.
Please note that we have issued a new registration certificates NR- 0502-D-107-G, NR-0502-D-108-G, AND NR- 0502-D-109 -G to replace NY-0502-D-107-G, NY-0502-D-108-G, and NY -
0502-D-109 -G, dated December 8, 2022 respectively. Copies of the new registrations are enclosed. Furthermore, please note that we are also working on issuing new registration certificates to replace your other NYSDOH certificates, and we will transmit these new certificates accordingly once they are complete.
Please note that some references are still missing. The NRC is currently awaiting for any additional background information for the registration certificates reference above. Please provide a copy as they become available.
Please be advised that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificate.
Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing should you decide to no longer manufacture, distribute, or offer service support for the product.
