Licensed Activities Prior to Submittal of NRC Form 241 Discussion:
I spoke with Mr. Clough about the statements in his e-mail of July 11, 2024, concerning the performance of licensed activities prior to the submittal of an NRC Form 241. He stated that there was a request in place for the job site (LRN 637610 Mansfield, CT) until July 5, 2024. He became ill prior to that date and did not return to work until July 11, 2024. Upon his review of the logs, he determined that the technicians took the portable gauges to the job site on July 8, 9, and 10, 2024. Mr. Clough informed me that he was the only person authorized to fill out the NRC Form 241s and submit them. Another individual has now been authorized to complete the NRC Form 241 in his absence should such an occasion arise again.
