30-Day Notification of Reportable Event The date, time, and exact location of the event 21 G-24-0113 GOV-01-55-04 ACF-24-0133 The event in question occurred on May 17, 2024, at approximately 1930 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.34365e-4 months <br /> (ET). Nuclear Fuel Services, Inc. (NFS) made the telephone report for this event on May 18, 2024, at approximately 1210 hours0.014 days <br />0.336 hours <br />0.002 weeks <br />4.60405e-4 months <br /> (ET). The event location is Building 303 at the NFS facility, located in the Town of Erwin, Unicoi County, Tennessee.

Radiological or chemical hazards involved, including isotopes, quantities, and chemical and physical form of any material released There were no radiological or chemical hazards associated with this incident.

Actual or p_otential health and safety consequences to the workers, the public, and the environment. including relevant chemical and radiation data for actual personnel exposures to radiation or radioactive materials or hazardous chemicals produced from licensed materials (e.g., level of radiation exposure, concentration of chemicals, and duration of exposure)

An operable Criticality Accident Alarm Station (CAAS) is required per 10 CFR 70.24. There were no actual radiological, chemical, or other nuclear safety consequences to the public, workers, or the environment. The potential consequences were that in the highly unlikely event of a nuclear criticality accident, evacuation could have been delayed for those personnel inside Building 303 resulting in a potential increase of the postulated doses during the event period.

The sequence of occurrences leading to the event. including degradation or failure of structures, systems, equipment, components, and activities of personnel relied on to prevent potential accidents or mitigate their consequences NFS has two speaker systems for Building 303 that are fully capable of sounding the CAAS alarm. On May 17, 2024 at approximately 1930 (ET), the redundant CIDAS speaker system was disabled to conduct an annual drill. At 2101 (ET) the CAAS alarm was sounded over the legacy speaker system and it was discovered the alarm was inaudible in Building 303. Compensatory actions were taken to shutdown processes, limit personnel, and provide temporary communication in the area. The redundant speaker system was restored to service at 2137 (ET),

then tested and confirmed to restore CAAS audibility in Building 303 at 2249 (ET). The total time that CAAS audibility was disabled was approximately 2.1

21 G-24-0113 GOV-01-55-04 ACF-24-0133 hours. On May 18, 2024 the legacy system speakers in Building 303 were restored to service after replacing the speakers and a portion of wiring.

The probable cause of the event, including all factors that contributed to the event and the manufacturer and model number (if applicable) of any equipment that failed or malfunctioned

The probable cause is partially damaged wiring between the alarm amplifier and speakers caused the speakers at the end of the zone to become inoperable and also prevented a trouble signal from being generated.

Corrective actions taken or planned to prevent occurrence of similar or identical events in the future and the results of any evaluations or assessments Immediate compensatory actions were taken to shut down processes, limit personnel access, and provide temporary communication in the area. The redundant speaker system was restored to service, then tested and confirmed to restore CAAS audibility in Building 303.

On May 18, 2024 the legacy system speakers in Building 303 were restored to service after replacing the speakers and a portion of the wiring. This restored normal public address capability and other alarm functions in the area. However, the alarm system capability to identify faults for this speaker zone was not restored by replacing the components. Daily function tests of the legacy speakers in Building 303 have been implemented until the fault sensing capability of the speaker zone is restored.

Troubleshooting and repairs to the remaining portion of the impacted legacy speaker zone are still in progress.
