Dear Mr. Kalas:
This letter is in response to your letter requesting to amend Sealed Source and Device Registration Certificate NR-1235-S-108-S dated January 10, 2024, to include a new annular PET cylinder source configuration, BMAN68-YY. The U.S. Nuclear Regulatory Commission staff has completed your request. A copy of the amended registration certificate is enclosed.
Please be advised that you must manufacture and distribute the products in accordance with the statements and representations contained in y our application, with enclosures thereto, and the information set out in your registration cert ificates. As a general rule, you must request and obtain an amendment to the certificates before you make changes or modifications to the information submitted to obtain the certificates.
Please read over the registration certificates in their entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing should you decide to no longer manufacture, distribute, or offer service support for the product.
