NRC REGULATORY CONFERENCE MEETING

SUMMARY

Licensee: Tennessee Valley Authority (TVA)

Facility: Sequoyah Nuclear Plant, Units 1 and 2 License Nos. DPR-77 and DPR-79 Docket Nos.: 05000327 and 05000328 Date of Conference: May 2, 2024 Meeting No.: 0240551 Enforcement Action: EA-24-008

On May 2, 2024, representatives of TVA and Sequoyah Nuclear Plant met with U.S. Nuclear Regulatory Commission (NRC) personnel in the Region II office located in Atlanta, Georgia, with some NRC staff attending virtually via Microsoft Teams. The purpose of the regulatory conference was to discuss the apparent violation (AV) documented in NRC Inspection Report Number 05000327/2024090 and 05000328/2024090, dated March 19, 2024 [NRC's Agencywide Documents Access and Management System (ADAMS) Accession No. ML24066A197]. The regulatory conference was held at the request of TVA and was characterized as an NRC Observation public meeting. Public participation in the meeting was via teleconference (dial-in). The list of attendees is provided as an enclosure to this summary.

The licensee and their representatives presented a summary of their interpretation of the conditions and factors resulting in the failure of the Sequoyah 1B Emergency Diesel Generator on September 19, 2023. While the licensee and the NRC share a common understanding of the facts associated with the event, the licensee does not agree with the agency interpretation and characterization of their meaning as captured within the finding and AV. In the March 19th inspection report, the agency preliminarily determined that improper restoration during a January 2023 maintenance outage of the rear exhaust valve bridge adjuster locknut and screw at engine 1B2, cylinder 14, caused separation and misposition leading to excessive global lash and valve failure. TVA and their representatives utilized the enclosed slide deck to provide an alternative qualitative analysis of the causal factors (ML24117A035). The licensee determined that an unpreventable internal spring failure in the outboard valve bridge assembly caused excessive local lash, which led to the subsequent loss of the outboard rear exhaust valve retaining ring.

While members of the public were invited there were apparent challenges with the Teams interface when time was permitted for them to provide their input. Members of the public were asked to provide their questions, comments, or concerns via the meeting feedback mechanism that can be found attached to the meeting notice on the public website or in ADAMS (ML24108A064). Following these interactions, the meeting was adjourned.

No feedback responses were submitted as of the date of this Regulatory Conference meeting summary.
