As I mentioned in my yesterdays voicemail to you, per the requirement of 10 CFR 70.38(j)
(1) and guidance contained in section 11.2 of NUREG-1757 Vol 1 Rev 2, please complete and submit the attached Form 314 for the decommissioned Lead Cascade Facility. This information is needed for our review of your November 2, 2023 request to terminate the Lead Cascade Facility license SNM-7003.
