On June 10, 2021, individuals at the MISTRAS Services location in Trainer, PA discovered a deviation in the calibration certificates for an acoustic emission instrument. This AE system was used for acoustic emission testing of llft rigs used for reactor head and internals. This deviation involved calibration certificates that were falsified by the individuals responsible for the acoustic emission tests being performed. The calibration certificates were intended to represent "calibration" performed by Physical Acoustics Corporation (AKA MISTRAS Products and Systems), but the "calibrations" were not performed as stated.
The 10 CFR Part 21 evaluation was initiated on June 15, 2021, and quickly indicated that the extent of condition extends to additional instruments and acoustic emission tests performed at the following nuclear plants:

The evaluation of this deviation Is essentially complete, and the documentation is being finalized. The final report will be m:ide available by November 1, 2023.
