Dear Mr. Levich:
Based on the information submitted in your application dated April 24, 2023, requesting a new registration certificate, we have completed your request. A copy of the registration certificate NR-1512-S-101-S is enclosed.
Please be advised that you must distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificates.
Please note that a certificate of registration, by itself, does not convey authorization for possession or distribution of radioactive sources or devices. A U.S. Nuclear Regulatory Commission (NRC) or agreement state license is required for possession and distribution of radioactive materials, sources, or devices. Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
Please be aware that only isotopes that meets the definition under Title 10 of the Code of Federal Regulations (10 CFR) Part 30.4 for byproduct material, are being added to the registration certificate. Isotopes including U-235, U-Natural, and Th-228 submitted in your application dated April 24, 2023, will need to be authorized under a license issued by the NRC or Agreement States under 10 CFR Part 40 or 10 CFR Part 70, and therefore, they were not included in registration certificate NR-1512-S-101-S.
You are obligated to notify us promptly in writing if you decide to no longer manufacture, distribute, or offer service support for the product.
