ISSUANCE OF NORDION (CANADA), INCORPORATED AMENDED SEALED SOURCE AND DEVICE REGISTRATION CERTIFICATE NR-0220-S-103-S
Dear Mr. Desgagne:
This letter is in response to Nordion (Canada), Inc. request dated January 24, 2023, requesting to amend Sealed Source and Device Registration No. NR-0220-S-103-S to for changes to the C-188 sealed source. A copy of the amended registration certificate is enclosed.
Please be advised that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificates.
Please note that a certificate of registration, by itself, does not convey authorization for possession or distribution of radioactive sources or devices. A U.S. Nuclear Regulatory Commission or Agreement State license is required for possession and distribution of radioactive materials, sources, or devices.
Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing if you decide to no longer manufacture, distribute, or offer service support for the product.
