WaterRemovalandInspection/Survey,approvedon1/20/2022.
TheladderthatleadstothebasementwasreplacedwithstairsusingChangeAuthorization
202203,StairInstallationinVBWRLowerLevel,approvedonJune6,2022.
AsbestosandLeadabatementworkwascompletedincontainmentonOctoberandNovember
2022,usingCA202204,AsbestosandLeadabatement,approvedonJune29,2022.
Structuralconditionsandradiationandcontaminationlevelsremainatacceptablelevels.
Environmentaldataismaintainedonsiteandavailableforreview.
2.0STATUSOFFACILITY
Inaccordancewithwrittenprocedures,theFacilityManagercontrolsaccesstothecontainment
buildingandgeneralsystems.Thefacilitycontinuestobeindeactivatedstatusinsafestorage
condition.
3.0ROUTINEPATROLS
Thefacilitywasinspectedweekly,quarterly,andannuallyinaccordancewithwrittenstation
procedures.
Patrolsidentifiedthefollowing:
x UnchangedEfflorescenceonthebasementwallsandfloor.
x UnchangedSpallingofconcrete,corrosionstainsandrebarcorrosionontheBasement
PumpFoundationpedestal.
x UnchangedSeepageofwaterintothebasementborelocation.

Theseconditionshavebeenevaluated,posenothreattostructuralintegrity,andarebeing
monitored.Thestructuralconditionofthefacilityisbeingactivelymonitoredandisstructurally
soundtoensuremaximumprotectionforpublichealthandsafety.

Completeradiationandcontaminationsurveysofthefacilityindicatethatlevelsremainlow.
Resultsofthesurveysarepresentedinattachment1.Airsamplingresultsarepresentedin
attachment2.Theradiation/contaminationlevelslistedarerepresentativebutnotnecessarily
maximumvalues.
AnalyticalresultsofgroundwatersamplesareavailableintheAnnualEnvironmentalReport

Inspectionoftheinstalledgaugesshowednopropagationofconcretecracking.

2.0STATUSOFFACILITY
Inaccordancewithwrittenprocedures,theFacilityManagercontrolsaccesstothecontainment
buildingandgeneralsystems.Thefacilitycontinuestobeindeactivatedstatus.Theplugstothe
reactorvessel,headstorageshield,andthewoodencoveroverthefuelstoragepoolremainin
place.
3.0ROUTINEPATROLS
Thefacilitywasinspectedweekly,quarterly,andannuallyinaccordancewithwrittenstation
procedures.
Threeissueswereidentifiedduringtheannualinspection:
- 1. Loose,falleninsulationwasfoundaroundtheperimeterofthemainfloorofEVESR.The
insulationisnotastructuralcomponentanddidnotcontainasbestos.
- 2. Corrosionwasidentifiedontheexposedrebarfloortieinextensionsthatarevisibleat
multiplelevelsofthefacility.Theseextensionsarenotstructuralcomponents.
- 3. Minimalefflorescencewasidentifiedatafewlocations.Thisconditionwasevaluatedand
doesnotposeastructuralissue.
