Report Date: March 11, 2022
Review the incoming report to determine if additional Commission or staff action is warranted. The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.
Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents little or no potential fo r radiation exposure.
- 1. The report identifies:
Significant reduction in the effectiveness of a package during use Defect with a safety significance x Shipment in which conditions of the approval were not observed.
- 2. What is the safety significance? High Medium X Low
- 3. Summary of the report:
In December 2021, GNF-A shipped a 30B cylinder, with a solid residual heel of enriched UF6, in the DN30 package (Docket No. 71-9362).
Upon receipt of the package by the consignee, it was noted that one of the six overpack locking mechanisms was not properly secured. The suspected apparent cause is that one of six pins was not fully engaged at 100% when the GN -FA operator torqued the final locking bolts. One pin was found to be about 20% out of the proper insertion position while its corresponding locking bolt had been properly torqued.
The integrity of the package was not compromised. The cylinder contained only solid heel quantities of UF6, the valve was fully functional, and the cylinder was leak checked prior to being shipped.
- 4. Corrective actions taken by the licensee:
The review of the operating procedure found that both the self-check and peer check reviews are not utilized. Actions were taken to strengthen the operating procedure and its corresponding log sheet to perform and document a peer check of this task. No other corrective action was necessary.

- 5. Staff comments:
This is a low safety significance event and there was no reduction in the effectiveness of the packaging as a result of this mistake. The partially engaged pin had no effect on the closure joint mechanical functionality: the joint was still functional with the pin 80% engaged.
The integrity of the package was not compromised. All other requirements of the CoC were met for this shipment.
- 6. Staff conclusion:
X The report does NOT identify generic design or license/certificate issues that warrant additional Commission or staff action. This report is considered closed.
There is a need to take additional action. Provide a summary of the bases and recommended actions:
