The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings are as follows:
- 1. Based on the inspection findings, no violations were identified.
- 2. Previous violation(s) closed.
- 3. During this inspection, certain of your activities, as described below and/or attached, were in violation of NRC requirements, and were assessed at Severity Level IV, in accordance with the NRC Enforcement Policy.
B. The following violation(s) is/are being cited in accordance with NRC Enforcement Policy. This form is a NOTICE OF VIOLATION, which may be subject to posting in accordance with 10 CFR 19.11.

Statement of Corrective Actions I hereby state that, within 30 days, the actions described by me to the Inspector will be taken to correct the violations identified. I understand that no further written response to NRC will be required, unless specifically requested.

- 16. Scope and Observations:
This was an unannounced, reciprocity inspection of a portable gauge/pipe profiler licensee out of Illinois working on the premises of Spartan Nash Distribution Center, 311 N Curry Pike, Bloomington, Indiana. The licensee is the manufacturer and, on this occasion, the user of the devices also. The device is a Lixi Model LS-82X with a 1.0 Ci Gd-153 source, used to detect corrosion on pipes underneath insulation. At the time of the inspection, a three-man crew utilized the device. The device is handheld, with an attached digital reader that displays a graph showing the measurement results. The source is shielded in a tungsten case equipped an on/off shutter, and is attached to a detector by a u-shaped rod; the device is very portable and lightweight. The source holder uses a collimator that projects a pencil-width beam directly to the detector, resulting in almost background radiation levels around and outside of the beam path when the gauge is used properly.
The licensee demonstrated how to use the gauge and how it was transported. The operators demonstrated an adequate level of understanding of emergency and handling procedures during interviews. The inspector reviewed leak test, training, dosimetry records, and the licensee's survey meter. The inspector took readings with its survey meter of the device being used, inches from the source, the highest reading was 140 microR.
No violations of NRC requirements were identified as a result of this inspection.
