HOPEWELL DESIGNS, INCORPORATED AMENDED SEALED SOURCE A ND DEVICE REGISTRATION CERTIFICATE NR-1138-D-101-S
Dear Mr. Vanderpool:
This letter is in response to the Hopewell Designs, Inc. applic ation dated February 3, 2022, requesting an amendment to NR-1138-D-101-S sealed source and de vice registration certificate for the G10 Series Irradiators to authorize the use of addition al source models. We have completed our review of your r equest, and a copy of the amended registration certificate NR-1138-D-101-S is enclosed.
Please be advised that you must manufacture and distribute any registered product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registr ation certificates. A registration certificate is meant to indicate that your product is acceptabl e for licensing purposes but does not authorize distribution or possession of radioactive materia l. As a general rule, you must request and obtain an amendment to the certificate before you m ake changes or modifications to the information submitted to obtain the certificate.
Please read over the registration certificate in its entirety a nd notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing if you decid e to no longer manufacture, distribute, or offer service support for the product.
