Request for Additional Information - Inactivation Certificate - U.S. Nuclear Regulatory Commission Date: Wednesday, June 22, 2022 12:52:00 PM Good afternoon Mr. Vanderpool, This email is in response to your letter dated May 28th, 2022, requesting the inactivation of registration certificates NR-1138-D-103-S and NR-1138-D-104-S. In order to continue with our review, we found that some additional information is required. Please address the item listed below:
In your letter you stated that 58 units of the UD794 model devices were distributed by Hopewell Design, Inc. If available, please provide the number of UD794 devices still in use, an estimate number is acceptable.
Please confirm receipt of this email and submit the requested information within 30 calendar days of the date of this email. If we have not received the complete information within 30 days, we will consider your application as having been abandoned by you. This is without prejudice to the submission of a complete new amendment request.
