Third Interim Notification per 1 O CFR Part 21 Degraded Snubber SF1154 Hydraulic Fluid Batch No. 18CLVS431

This letter is a follow up to my letter dated March 3, 2022 pertaining to the above subject.

The following table reflects the status of US facilities to which SF1154 fluid from Batch 18CLVS431 was shipped by Lake Engineering Company (LEC) in accordance with purchase orders which specified that the requirements of 10 CFR Part 21 are applicable.

FACILITY STATUS RE: BATCH 18CLVS431 FLUID COMMENTS

Waterford 3 Fluid used in some installed snubbers No substantial safety hazards

ANO Use status to be determined by facility No substantial safety hazards

Beaver Valley Fluid used in some installed snubbers No substantial safety hazards

So. Texas Project Fluid used in some installed snubbers No substantial safety hazards

Sequoyah Fluid not used in installed snubbers No substantial safety hazards

NTS Use status to be determined by facility Evaluation ongoing

Seabrook Fluid used in some installed snubbers No substantial safety hazards

Lake Engineering Company shall continue its evaluation to determine whether the subject anomalies represent defects which could create a substantial safety hazard. Determination in this regard shall continue to involve interaction between Lake Eng1neenng Company and the facilities listed above.

Our current goal is to complete our evaluation by July 12, 2022 at which time a determination will have been made as to whether a substantial safety hazard exists. If additional time is required for the above determination, an interim report shall be submitted that shall include an updated completion date.
