The tent also tore on the east connection to the base. The tent then flapped in the wind. Small sections of the tent blew off after the initial tear. There was no one in the tent at the time it tore.

There were no injuries as a result of the tear. Wind speeds at the time (Waukegan) were 25 mph with gusts to 45 mph from the west.

Title:

U2 Construction Tent Damaged in High Winds CR Significance Level:

2 Evaluator: Robert Keene Date: 04/12/2017 I. Problem Statement:

Relatively high wind speeds were encountered at Zion on 03/08/2017. Due to a failure mechanism in the Unit 2 Containment construction tent, half of the main covering ripped and became dislodged from the support structure.

II.

Background

/ Analysis:

On Wednesday March 8, 2017, strong gale force and storm winds developed at the site causing heavy damage to the Unit 2 Containment construction tent. The wind speeds at 0600 were greater than 20 miles per hour (mph) for sustained winds and greater than 30 mph for wind gusts. The Unit 1 and Unit 2 tents and the tent at the Waste Water Treatment Facility (WWTF) were observed throughout the morning for any damage due to the heavy wind. Unit 1 had developed two slight rips in the tent fabric at the top of the roll-up door (West side of the tent) at the rail opening and no issues were observed at the Unit 2 tent or the WWTF. Personnel proceeded to hot patch the two rips in the Unit 1 tent in accordance with manufacturer specifications then the entire site stopped work for lunch.

At approximately 1130 the Unit 2 tent fabric failed. The East side of the South half of the tent started to tear free from the support structure. The fabric failure continued to propagate such that only half of the fabric remained on the structure in this area.

Sustained winds were at 30 to 40 mph with gusts as high as 56 mph.

When work resumed after lunch, all crews were instructed to place equipment and work areas in a safe condition and assemble back in the break areas. Work onsite was then stopped for the remainder of the day. The area around the Unit 2 tent was roped off and all personnel were instructed to stay clear of the area. A small group of personnel were kept onsite to perform inspections and possible repairs as necessary.

Due to the condition of Unit 2 tent, no personnel entered the area to perform further inspections. After discussions with all applicable work groups, hazard assessment and pre-job brief a small crew was dispatched to Unit 1 tent to make adjustments to ratchet straps and inspect prior repairs. Upon completion of work in Unit 1 work all work was stopped at approximately 1545.

On Thursday March 9, 2017, just after midnight the wind subsided and no further damage was noted on any of the tents. The north half of the Unit 2 tent remained secure and undamaged. There was no primary structural damage to the Unit 2 tent framework.

There was some minor damage to secondary structural supports (wires, turn buckles and

IR-2017-0038 straps) due to the loose fabric blowing in the wind and impacting these areas. The piece of fabric that failed was considered a total loss and required replacement.

On Thursday March 9, 2017 the site was inspected and walked down prior to any work commencing. A competent person completed an extensive inspection and found no compromise of the primary tent structural integrity. The Unit 2 tent fabric assembly was found to be compromised and torn due to the wind. All work areas and both tents were released to continue limited approved work. Unit 2 tent was limited to not allow for the temporary storage and loading of contaminated waste until the fabric is fully repaired.

Unit 1 tent resumed normal activities with a limitation on the amount of contaminated waste temporarily stored until adequate corrective actions are put in place. The amount limited was such that in the case of sustained winds anticipated in excess of 25 mph, all staging of waste in the Unit 1 tent is stopped and all waste within the Unit 1 tent is to be loaded into suitable waste containers by the end of the work day.

Upon inspection of the Unit 2 tent and surrounding area for hazards, more property damage was identified. There was minor damage done to the dispensing hose on the Unit 2 Diesel Fuel Storage Tank located on the East side of the Unit 2 tent. The nipple from the dispensing hose to the pump was sheared off when a conduit pipe at the bottom of the tent fabric fell on it. A small amount of leftover fuel from the dispensing hose was lost to the ground and was immediately absorbed and disposed of in the appropriate containers.

On Thursday March 9, 2017 Radiation Protection performed extensive surveys of outside areas around the Unit 2 tent and the entire East side of the property. All scraps of the tent that had ripped off the prior day were collected, surveyed and disposed of.

No contamination was identified on the ground or the pieces of tent. Air sampling was in place inside and outside of the Unit 2 tent at the time of the event as well as the perimeter air samplers in support of Auxiliary Building/Fuel Handling Building Open Air Demolition. All air samples were analyzed on 03/09/2017 and no airborne activity was identified. For the couple of weeks prior to the event, there had been radioactive material moved into or out of the Unit 2 tent. Also, there was no contaminated waste being stored inside the Unit 2 tent at the time of the event. Daily surveys were reviewed for the prior 2 weeks and no contamination was identified. The daily survey was performed in the morning of 03/08/2017 on the day of the event prior the tent failure and no contamination was identified. A posted contaminated area was present in the tent extending out from the Unit 2 Containment opening to accommodate surveys and movement of equipment. All surveys of this area for the prior 2 weeks also did not identify any radioactive material. Over the next few days, more extensive surveys of the contamination area were performed, no contamination was identified and the contaminated area was down-posted and the boundary moved back to the opening of containment. Based upon all of the radiological data, there was no release of radioactive material to the environment as a result of this event.

IR-2017-0038 The construction of the Unit 2 tent was performed by site personnel under the direct supervision of Clearspan employees along with MBI and ZS oversight personnel. The Unit 1 tent was constructed by the same site employees and MBI oversight personnel without the direct supervision of Clearspan employees. After the event, Clearspan was contacted and confirmed the tents were designed to support 20 pounds per square foot of snow load and 80 mph of 3 second wind gusts per the international building codes.

Available data for maximum wind speeds did not report wind speeds in excess of this 80 mph limit. The results of the full investigation as to why the fabric on the Unit 2 tent failed is believed to be the result of an existing unseen abrasion or small tear in the fabric that occurred before the wind storm or an undetected defect in the fabric during the manufacturing process. With over one half of the tent material ripped off into pieces after the storm it was difficult to ascertain the exact cause of the fabric failure.

On March 16, 2017, two Clearspan employees arrived at Zion to perform a structural inspection of the Unit 1 and Unit 2 tents in concert with MBI and ZS employees. The Clearspan employees assisted with minor adjustments to sway cables and ratchet straps.

The Clearspan Truss Arch Specialist supplied documentation to MBI confirming the structural integrity of the tents. He also stated that all of the existing cables and ratchet straps were tight and in good working condition.

Corrective actions being implemented by MBI are to perform weekly inspections of the inside and outside of the Unit 1 and 2 CNMT tents and the WWTF tent fabric, all of the structural components, sway cables and ratchet straps. Additionally, inspections and adjustments to both the Unit 1 and Unit 2 tents will be performed and after any expected high winds or severe weather. These actions were implemented the week of March 20th and the waste staging/loading temporary restrictions in the Unit 1 tent were lifted.
