Based on the information submitted in your amendment request da ted June 16, 2021, requesting an amendment to your Sealed Source and Device Regist ration Certificate NR-1494-D-101-B to add two additional models, we have completed our rev iew of your request. A Copy of the Registration Certificate NR-1494-D-101-B is enclosed.
Please be advised that you must distribute the product in accor dance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you m ust request and obtain an amendment to the certificate before you make changes or modific ations to the information submitted to obtain the certificates.
Please note that a certificate of registration, by itself, does not convey authorization for possession or distribution of radioactive sources or devices. A U.S. Nuclear Regulatory Commission license is required for possession and distribution of radioactive materials, sources, or devices.
