Conduct an annual onsite inspection of all NRC AEDs located at the locations referenced in Section 3.0, Scope of Work; if equipment and/or supplies is set to expire prior to the next site assessment, the contractor shall replace the equipment and/or supplies prior to the expiration date if it occurs before the next site assessment. Providing medical direction/oversight for the program; Ensuring and providing physician prescription for each AED at each location.

Providing and/or maintaining written protocols, policies and procedures; Tracking and monitoring inventory of equipment and accessories, including warranty information shall be provided at quarterly discussion with NRC stakeholders. Recommends being submitted after the site assessment is conducted (Reference Task 1)

Replacing AED supplies as the supplies expire or are used.

Advising of necessity for the purchase/replacement of AED equipment, accessories and supplies, including product and manufacturer recalls; Providing CPR/AED training for NRC employees located at Headquarters, Regional Offices, and the Technical Training Center; Conducting post-event data review with report submission by qualified medical personnel; Assisting with any required Government paperwork/documentation; Post Award Meeting The Contractors Program Manager, Contracting/Business representative and key personnel shall attend a post award kick-off meeting within five (5) business days after contract award.

SPECIFIC TASKS Task 1: AED Program Management Services

- Medical Oversight and Medical Direction Requirement The Contractor shall provide medical oversight and medical direction for the NRCs AED Program, which will consist of a range of services and advice to include site surveys, AED equipment recommendations, and recommendations for new or updated emergency response protocols for all NRC locations. The Contractor shall provide assistance and support to the NRC to educate employees on the use of AEDs and to assist the agency in its emergency preparedness related to the use of AED equipment.

At the request of the NRC Contracting Officer Representative (COR) the contractor shall perform a site survey to assess the current level of preparedness, number of AEDs suggested for each location and the most strategic placement of these devices. The Contractor shall 6

V797P221D/NRC-HQ-84-17-T-0001 M00011 develop and provide NRC Medical Direction Protocols based on the site and capability.

Protocols developed may be unique to each facilitys physical and environmental structure.

If an incident were to occur requiring the use of an AED, upon direction of the COR , the Contractor shall be responsible for downloading the data from the device, have certified and trained medical personnel review the data, critique the response plan, provide an official post-event review report that includes the time, duration, and outcome of the event, and provide suggestions for improvement or an After Action Plan within 30 days. The Contractor shall ensure a replacement AED is provided while the used AED is out of service.

Standard The Contractor shall provide qualified personnel with the necessary education and experience to offer the full range of services described in this Statement of Work. The Contractor shall utilize qualified emergency medicine physicians for medical direction tasks.

Acceptance Criteria No more than one instance of not meeting the standard during a six-month period.

Performance Incentives There will be a one percent deduction from the contractors invoice for this CLIN for non-compliance with the standard.

Deliverables The Contractor shall provide written site surveys, recommendations for medical direction protocols and emergency response procedures, and post-event review reports as requested by theCOR.

- Equipment, Maintenance, Calibrations, Repairs, and Replacements Requirement The Contractor shall maintain, inspect, and repair all AED equipment in NRC offices and facilities. The Contractor shall perform an annual onsite inspection of AED devices to ensure proper functioning. The Contractor shall be responsible for tracking expiration dates on pads, batteries, and accessories, as well as device warranty expiration or recall. The Contractor shall replace supplies as the supplies are expired or are used. The Contractor shall maintain this information on behalf of the NRC and provide the Data Inventory Report in the form of an excel file report to the NRC via E-Mail.

Standard The Contractor, as part of the quarterly discussion with NRC stakeholders, shall make cost-effective recommendations for maintenance, replacement and/or purchase of additional AED equipment and supplies, providing at least three (3) vendors and price lists to the NRC COR ,

Contracting Officer, and Contract Specialist prior to replacement of equipment. The Contractor 7

V797P221D/NRC-HQ-84-17-T-0001 M00011 shall perform an annual onsite inspection of all AED devices at NRC Headquarters, Regional Offices, and the Technical Training Center.

Acceptance Criteria No more than one instance of not meeting the standard.

Performance Incentives The contractor will receive an adverse past performance rating on Contractor Performance Evaluation.

Task 2: Training Classes Requirement The Contractor shall provide CPR/AED training for NRC employees as requested by theCOR .

It is anticipated there will be 15 training classes per year. The Contractor shall be responsible for tracking staff trained and expiration of certifications and maintain this information on behalf of the NRC. Upon completion of each training class, the Contractor shall provide a report to the COR that contains the following information: class title, class date, employee name (first/last),

CPR certification date, CPR certification expiration date.

Standard The training provided will be in accordance with American Red Cross and American Heart Association guidelines for CPR/AED training. The Contractor shall provide reminders to the COR 4 to 6 months in advance of certification expirations. The Contractor shall provide training class reports to the COR within 5 calendar days from completion of each training class.

Acceptance Criteria No more than one instance of not meeting the standard.

Performance Incentives The contractor will receive an adverse past performance rating on Contractor Performance Evaluation.

3.1 Supervision/Management The Contractor shall prepare work schedules for Contractor employees, supervise and control the method by which the Contractor performs the service. The Contractor shall define the specific duties of individual Contractor employees.

3.2 Personnel Requirements The Contractor shall provide the NRC with qualified personnel to perform the services described in this Statement of Work. This will include AED Program medical oversight and training at each major NRC location. The Contractor shall ensure that all its employees, subcontractor 8

V797P221D/NRC-HQ-84-17-T-0001 M00011 employees or consultants who are assigned to perform work herein for contract performance for periods of more than 30 calendar days at NRC facilities, are approved by the NRC for unescorted NRC building access.
