001 pursuant to the requirements of Technical Specifications (TS) 3.3.3.10 Action a and TS 6.9.2. This special report is required by VCSNS TS because Loose Part Monitoring System (LPMS) Channel 8 has been inoperable for greater than 30 days.
LPMS Channel 8, monitoring Steam Generator (SG) "C," was declared inoperable on February 24, 2021 due to erratic operation, ostensibly caused by an intermittent connection in the Channel 8 signal transducer (IAT00758) circuit. Channel 8 has been disabled in LPMS. However, SG "C" LPMS Channel 9 continues to monitor SG "C" for loose parts, only without Channel 8 coincidence. Loss of dual channel coincidence merely reduces one aspect of the false alarm filtering functionality. The failed transducer (IAT00758) is scheduled for repair or replacement during Refuel 26 in Fall 2021.
