Thank youfor reviewing ourlicense request.Inresponse toyour request dated January 19, 2021, we provide the followingadditionalinformation:

1. Niowave confirms that leak testsforsealed will be performed atintervals sources approved bythe NRCor anAgreement State andspecified intheSealed Source and Device Registration Certificate. Leak tests will beperformed byanorganization authorized bythe NRCor an Agreement State toprovide leak testing services toother licensees. Leak maybecollected tests bythe licensee using a leak test kit supplier's instructions.Such leak testkits will besupplied byanorganization authorized bythe NRCoranAgreement Statetoprovide leak testingservices.

2. Niowave confirms that physical inventories willbeconducted atintervals nottoexceed 6 months, to account for allsealed sources anddevices received and possessed under thislicense. Records of inventories willbemaintained for 3years from thedate ofeach inventory and willincludethe radionuclides, quantities, manufacturer's name, model numbers, andthe dateofinventory.

3. Niowave confinns that pursuant to10CFR70.25(g) and10CFR70.51(b)(3), we will maintain records important todecommissioning andtransfer theserecords NRCorAgreement to an State licensee before licensed activities are transferred orassigned, inaccordance with 140CFR70.36. Furthermore, pursuant to 10CFR70.51(a)(3), we will forward the records required by10CFR70.25(g) tothe appropriate NRC regional officebefore the licenseisterminated.

4. Niowave confinns that we will usethe decay-in-storage disposal wasteprocedure that ispublished in Appendix P inNUREG-1556, Volume 7,Revision 1,"Program-Specific Guidance About, Academic, Research andDevelopment, andOther Licenses with Limited Scope" for the disposal ofradioactive waste containing only isotopes withhalf-livesthat areless than 120days.
