-(iv) Nature of the defect or failure to comply and the safety hazard which is created or could be created by such defect or failure to comply.
Duke Energy, based on a 10CFR Part 21 notification from Lake Engineering, notified Enertech of the issue related to silicon fluid, Batch 17BL VS293. The potential defect identified in the Fluid to have opaque semi solid material. The fluid used in various snubbers. In snubbers, the semi-solid material could potentially block the bleed orifice and cause the snubber to lock up.

CW Enertech procured (3) 200 kg (55-gallon drums) on December 18, 2017 Momentive Batch 17BLVS293. This was separated into smaller 5-gallon container and supplied to the following sites:

We understand the potential hazard in regards to the snubber-fluid, however Enertech no longer has any fluid remaining for thi$ batch. *we then informed our customers the fluid was reported to NRG and we are not able to test the fluid and cannot determine if the fluid will have any impact on the safety function of the snubber.
