NRC INTEGRATED INSPECTION REPORT 50-352/98-03,50-353/98-03 -

. Dear Mr. Rainey:

This refers to the inspection conducted on March 17,1998, through May 18,1998, at

lyour Limerick Units 1 and 2 reactor facilities. The enclosed report presents the results of

this inspection.

During the 9-week inspection period covered by this report, the conduct of activities at

Limerick Units 1 and 2 was generally characterized by safe and conservative operations

~

and decision making.' In particular, your operators' response to the potentially hazardous

environment in the plant that resulted in an ALERT classification was very good. ~ In general

your staffs control and oversight of refueling activities was excellent.

However, we are concerned that your operators did not enter Technical Specification 3.0.3

when both control room emergency fresh air systems were declared inoperable and this

decision wa<. supported by operations management.' Further your assessment of the

control room emergency fresh air system related issues did not identify this decision as a

problem nor identify the technical specification limiting conditions for operation entries that

- were missed.

The Radiation Protection program was found to be generally effective. Dose reduction

. efforts due to drywell shielding and hot spot flushing were effective. Continued refinement

. is warranted in electronic dosimeter alarm setpoint use and in providing effective RP

oversight and leadership in the advanced radiation worker (ARW) program.

Based on the results of this inspection, the NRC has' determined that a violation of NRC

requirements occurred. This violation is cited in the enclosed Notice of Violation (Notice)

- and the circumstances surrounding them are described in detail in the subject inspection

_

report. The violation is of concern because it involves inaccuracies in the control room log
which is used, in part, for configuration control of plant systems. You are required to..

respond to this letter. and should follow the instructions specified in the enclosed Notice

'when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory
