TRANSPORTATION OCCURRENCE - LOST SOURCE (116 Ci Ir 192)
Automation Industries, Phoenixville, Pa. reported to our Region I office that a package weighing 40 lbs. shipped via Federal Express Co., Memphis, Tenn.,
on January 9, 1976 to Advex Co., Hampton, Virginia containing an exposure device with a 116 Ci Ir 192 source has been lost in transit. Automation requested a trace of the package.
Federal Express, advised that'the trace was unsuccessful. The shipment consisted of two separate packages, one containing the controls and one containing the source.
The package containing the controls was received by Advex Co. on January 10, 1976.

The States of Pennsylvania and Virginia have been informed. A press release is not planned by the licensee, however, NRC is i
drafting a press release for consideration.

Memphis, Tenn. and Moore Trucking Co., Lansdale, Pa. and has requested the g
initiation of another search of all involved terminals and warehouses.
A Region I, Philadelphia is continuing an active inquiry into this matter.
