MODIFICATION NO. 3 TO TASK ORDER NO. 2 ENTITLED *lNSPECTION REPORT CODING INTO HFIS" UNDER CONTRACT NO. NRC-02-95-003 The purpose of this modification is to: (1) increase the level of effort by 1,092 professional hours, (2) revise the scope of the basic task order to DELETE performing data entry for inspection reports into the HFIS, and INCREASE the number of months the Contractor will be required to review and code inspection reports from "12" to "24" months. All work shall be performed in accordance with the attached Statement of Work, (3) change the titlie of Task Order No. 2 from "lNSPECTION REPORT CODING AND DATA ENTRY INTO HFIS" to

  • lNSPECTION REPORT CODING INTO HFIS", (4) extend the expiration date of the task order from " September 30,1998" to " June 5,1999" to allow for the completion of the review and coding of inspection reports for the additional 12 months, and (5) increase the ceiling amount by $73,287.46 from $87,131.00 to $160,418.46, and (6) obligate additional funds in the amount of $27,821.00, thereby increasing the obligated amount of the subject task order from

$87,131.00 to $114,952.00.

In accordance iTvith Section 5.4(c) of the subject contracrentitled " Task Order Award," this letter F ~

definitizes Modification No. 3 to the subject task order.

The second paragraph of the definitization letter of the basic task order is deleted in its entirety and substituted with the following in lieu thereof:

i "The period of performance for Task Order No. 2 is from September 18,1997 through June 5,1999. The total cost ceiling is $160,428.48, of which the sum of $149,923.95 represents reimbursable cost, and the sum of $10,494.51 represents fixed fee."

2 Backaround 0ver the past several years, the NRC Human Factors Assessment Branch has E

implemented the Human Factor Information System. The system now contains seven modules. one of which details human performance information extracted from inspection reports.

The data base currently contains human performance inspection report data from 1994 to the present.

Obiective The, objective of this task order is to obtain assistance from OrTech for the p -._

l review and coding of human performance issues _in 24 months of inspection reports using the HFIS Inspection Report Coding Instructions Technical and other Soecial Qualifications Reauired One nuclear specialist, with expertise in evaluating human performance, for review and coding of human performance issues in Inspection Reports.
