Completion of Actions Requested for Generic Letter 96-01, " Testing of Safety-Related Logic Circuits," for the Davis-Besse Nuclear Power Station Ladies and Gentlemen:
This letter confirms the completion of Toledo Edison's actions as requested by Generic Letter 96-01,
" Testing of Safety-Related Logic Circuits," for the Davis-Besse Nuclear Power Station (DBNPS). On January 10,1996, Generic Letter 96-01 requested licensees take the following actions:
- 1) Compare electrical schematic drawings and logic diagrams for the reactor protection system, eme gency diesel generator load shedding and sequencing, and actuation logic for the engineered safety features systems against plant Surveillance Test procedures to ensure that all portions of the logic circuitry, including the parallel logic, interlocks, bypasses and inhibit circuits are adequately covered in the Surveillance procedures to fulfill the Technical Specification requirements. This review should also include relay contacts, control switches, and other relevant electrical components within these systems, utilized in the logic circuits performing a j
safety function.

- 2) Modify the Surveillance procedures as necessary for complete testing to comply with the Technical Specifications.
Completion of these actions was requested to be accomplished prior to startup from the first refueling outage commencing one year after the issuance of the Generic Letter. In a letter dated April 16,1996, (Serial Number 2370), Toledo Edison committed to completing this review prior to startup from the
. eleventh reseling outage, which began on April 10,1998.
In order to co.aplete the requested actions of Generic Letter 96-01, a review group of knowledgeable engineering personnel was established to perform a methodical review of the circuitry and associated testing utilized to satisfy each required DBNPS Technical Specification Surveillance Requirement. A review ruethodology was developed by the review group and approved by DBNPS management to assure a consistent and documented review was performed. - SerialNumber 2542 Page 2 number ofissues affecting the review pursuant to Generic Letter 96-01 provided by the Nuclear Regulatory Conenission at the Atlanta workshop held on March 19,1996.
Any Surveillance Requirement that involved the' automatic actuation of at least one component or system was included in this review, except when the automatic actuation follows a manual action. This resulted
. in review of logic circuitry for the reactor protection system, emergency diesel generator lead shedding and sequencing, and actuation logic for the engineered safety features systems as required by the Generic
. Letter. Also included in the review was logic for the anticipatory reactor trip system, the steam feedwater rupture control system, the emergency core cooling systems, the auxiliary feedwater system, the component cooling water system, the service water system, and the containment air cooling system.
When a possible Surveillance Requirement non-compliance was identified by the review group, it was presented to a multi-discipline review board comprised of permanently assigned representatives from the Operations, Maintenance, Engineering, and Regulatory Affairs Departments. When this review board determined that a Surveillance Requirement was not being adequately fulfilled, a corrective action
. document was initiated, the operability of the subject equipment immediately evalcated, and the appropriate actions taken in accordance with the DBNPS Technical Specifications to restore equipment I
operability, if this res iew board could not reach a consensus on a specific issue, the issue was elevated to the DBNPS onsite management review committee (the Statioa Review Board). Eighteen conditions were identified during this review process where Surveillance Requirements were not being adequately fulfilled, and these conditions were reported in accordance with 10CFR50.73 under Licensee Event Report 97-008, Revisions 00 through 13. All of the testing performed in response to these deficiencies revealed the circuitry functioned as designed with no equipment malfunctions.
. The review of the Surveillance Requirements for the logic circuitry described above has been completed,
. and all required Surveillance procedure changes were completed on or before May 13,1998, prior to the conclusion of the eleventh refueling outage on May 23,1998. These actions complete the actions requested by Generic Letter 96-01 for the DBNPS. The review program also identified other potential enhancements for testing oflogic circuits, which have been documented for future evaluation and potential incorporation into DBNPS's Surveillant Test Program.
