SUBJECT: DAVIS-BESSE INSPECTION REPORT 50-346/98005(DRP) AND

NOTICE OF VIOLATION

Dear Mr. Wood:

On May 12, i998, the NRC completed an inspection at your Davis-Besse site. The enclosed

report presents the results of that inspection.

The inspection mainly reviewed activities conducted during the eleventh refueling outage.

Generally, these activities were well planned and executed, and shutdown risk controls for

evolutions were effective. During the reactor shutdown for the outage, the operators effectively

responded to an unexpected failure of a purification demineralized. Engineering, maintenance,

and health physics support was also effective during the outage.

However, during the outage several events involving crane and rigging opervions inside

containment occurred that jeopardized equipment and personnel safety. We understand that

station personnel are assessing the collective significance of the events.

In addition, one violation of NRC requirements was identified during the inspection. The violation

pertains to inadequate procedures for the control of temporary manifold isolation valves. This

violation is of concem because adequate instructions were not provided to ensure all valves l

were properly manipulated on a temporary manifold. [ l

The violation is cited in the enclosed Notice of Violation, and the circumstances surrounding the '

violation are described in detail in the enclosed report. Please note that you are required to I

respond to this letter and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to determine whether further

enforcement action is necessary to ensure compliance with regulatory requirements.
