. SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-277/98-02,50-278/98-02;

NOTICE OF VIOLATION

Dear Mr. Rainey:

On May 4,1998, the NRC completed an inspection at the Peach Bottom Atomic Power

Station, Delta, Pennsylvania. The enclosed report presents the resutts of that inspection.

During the severe a .< period covered by this inspection period, the conduct of activities

L at the Peach Bottom facility was generally characterized by safety-conscious operations

and well-coordinated maintenance activities. We also noted good engineering support of

plant operations.

Based on the results of this inspection, the NRC has determined that two violations of NRC

requirements occurred. These violations are cited in the enclosed Notice of Violation

(Notice) and the circumstances surrounding them are described in detail in the subject

inspection report. The first violation involved a high pressure coolant injection (HPCI)

system operating procedure that did not provide aGequate instructions regarding the HPCI

pump turbine vibration monitoring system. The se cond violation was the failure of health

physics versonnel to follow radiation area control grocedures regarding posting of an open

door to a potential high radiation area. Both of the violations were NRC-identified.

We are also concerned about a number of instances of plant valves being identified out of

[' _ their required or expected position. Although several of these valves were in non-safety

related systems, three valves were in safety related systems. We determined that, taken

collec+ively, these items represented a weakness in plant status control. Some of the /

investigations for these issues were still in progress at the end of our inspection. [

Accordingly, we have opened an unresolved item to review the results of your j

investigations into the7 mispositionings. /

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
