Dear Mr. Randolph:

The purpose of this letter is to annour, e our medium-and low-voltage breakers inspection at your Callaway Plant reactor facility. This inspection will be conducted in accordance with Temporary Instruction 2515/137, Revision 1, " Inspection of Medium-Voltage and Low-Voltage l

Power Circuit Breakers." We have scheduled this inspection for July 6-10,1998. This inspectior: will involve a team of four NRC personnel.

The inspection will assess medium-and low-voltage switch gear maintenance activities; refurbishment and lubrication methods; programs to address NRC generic communications and manufacturers' generic communications; lessons leamed from industry operating experience; dedicstion of commer ial grade procured components; your treatment of breakers in the Maintenance Rule prograrn; breaker maintenance history; and breaker reliability.

We request that you arrange a short presentation following the entrance meeting regarding the subject breakers installed at your facility. The inspection team will require information about your program and its implementation for in-office preparation for the inspection. A description of the needed material is in the enclosure to this letter. Our designated team leader (Paul C.

Gage) will be contacting your staff and arranging for some of this specific material to be available to the team by June 24,1998. If you have any questions conceming this inspection, please contact Mr. Gage at 817/860-8273 or me at 817/860-8195.

A listing of all safety-related medium-voltage and low-voltage metal enclosed circuit breakers along with the following information for each breaker:

Serial number, if assigned (unique identifier, not just shop order number)

Cubicle identification number where currently installed Original equipment supplier (if different from manufacturer) in-service date Date most recent preventive maintenance was performed l

Date of most recent complete overhaul or refurbishment Breaker functional failures, whether in-service or during testing in last 5 years, if any. Give root cause(s) and corrective actions (s)

Any other corrective maintenance in last 5 years (i.e., to correct unsatisfactory conditions discovered before in-service failure)

Updates, upgrades, or modifications Whether included in the scope of the Maintenance Rule program 2.

Plant procedures for general or corrective maintenance on the breaker (if separate from No. 3 below) 3.

We prefer to review original records and will request copies, as required, unless records are archived and must be printed from microfilm or microfiche.

For the purposes of this inspection, " refurbishment" (or overhaul or remanufacture) means major maintenance that may include the following:

Complete predisassembly inspection and testing (documenting as-found results);

(2)

Complete post-use disassembly; i

Inspection of parts for damage or wear, lubricant, proper type, dirt or evidence of other contaminants; (4)

Replacement of worn (out-of-tolerance) or damaged parts; (5)

Cleaning, relubrication, represervation, reassembly, adjustment, final inspection and testing; (6)

Any data for trending key operating parameters, such as timing, forces, dimensions and adjustments, reduced control voltage tests, pole or contact resistance, insulation resistance (including both mein poles and control components, etc.); and 1
